Club Recognition Award

Any current club volunteer can be selected at the discretion of their local club's leadership.

The regional club board is encouraged to establish a nomination and selection process which can be incorporated into the club’s bylaws.

• Submit this form to specify details for the standard Club Award.
• Plaques being sent domestically cost $65 and include shipping and handling.
• Plaques being sent internationally cost $85 and include shipping and handling.
• Please allow 4-6 weeks to process awards for domestic clubs; 6-8 weeks for international clubs.
